CAB HEALTH AND RECOVERY SERVICES, INC.
BENEFITS SUMMARY
Our employees know that working at CAB Health & Recovery
Services means more than just a paycheck and a job. Instead, we
offer a generous benefit package which allows you to take time
and care for yourself and your family.
In spring 2006, we were designated as a “Family Friendly
Business” by the American Association of University Women (North
Shore branches, Mass.). The designation is given to
organizations which support working women, provide generous
family-friendly policies and which work to improve our
communities.
Note: These are just highlights of your expected benefits as an
employee. It does not cover all you need to know about each
benefit. For detailed information, you may ask at your employer

Earned Time Off (ETO)
Eligibility:
Employees who are scheduled to work at least 20 hours per week.
Description:
ETO combines traditional vacation, paid vacation, and paid sick
leave into one flexible paid-time-off policy. Accrual rate is
determined by how long you have worked at CAB and your regularly
scheduled work hours per week.
New employees cannot use ETO until six months of
benefit-eligible employment, except for agency recognized
holidays. ETO is fully vested after six months of employment.
Health Insurance
Eligibility:
Employees working 32 regularly scheduled hours per week. You are
eligible for health insurance coverage on the first day of the
month following your month of hire. New employees must enroll in
a plan within the first 30 days of employment or during CAB's
annual open enrollment period in November of each year.
Tufts HMO plan is a referral system designed to
provide you with access to health care directly from within the
Tufts network of health care providers. You are required to
select a primary care physician (PCP). Employees electing health
insurance coverage must authorize a payroll deduction for their
portion of the premium. The portion of the premium paid by the
employee varies with the type of coverage selected.
Dental Insurance
Eligibility:
Employees who are scheduled to work at least 20 hours per week
are eligible on the first day of the month following date of
hire. You must enroll within the first 30 days of employment or
during the June annual enrollment period.
Description:
The voluntary dental option is a managed fee for service plan
that allows you to pick your dentist from more than 5,000
dentists in Massachusetts. This plan is designed to offer you a
broad range of services - while helping to reduce your dental
costs.
Disability Insurance
Eligibility:
At the completion of one year of service, employees who are
scheduled to work at least 32 hours per week are eligible for
employer paid short-term and long-term disability insurance.
Description:
Short-term disability pays a range of 100% of annual earnings up
to $500 per week to 60% of annual earnings up to $300 per week.
Retirement Plan
Employee contributions:
You may choose to make tax deferred contributions to the
retirement plan through payroll deduction.
Employer contributions:
Employer contributions are discretionary and may be made
annually, based upon eligible employee paid compensation for a
specified period. Employees who are at least 18 years old, have
worked for CAB Health & Recovery Services, Inc. for at least one
year, and have worked 1,000 hours or more in the year being
funded are eligible to participate. Employer contributions are
fully funded after they have completed 3 years of service.
Life AD&D Insurance
Eligibility:
If you work 32 hours per week and after one year of service, you
are eligible for employer paid life and accidental death and
dismemberment insurance.
Description:
Life accidental death & dismemberment insurance is worth 1X
basic annual earnings up to a maximum of $80,000.
Flexible Spending Account:
Eligibility:
Employees who work at least 20 hours per week are eligible
within the first 30 days of hire or during our open enrollment
period in November.
Description:
The reimbursement account (sometimes called an FSA) allows you
to set aside money on a pre-tax basis to cover medical or
day-care expenses for yourself, your spouse, and all dependences
claimed on your tax return.
AFLAC
Eligibility:
You are eligible to sign up within the first 30 days of hire or
during our open enrollment in November.
Description:
AFLAC offers supplemental insurance including accident, cancer,
and life insurance. These options are done through a payroll
deduction. However, upon leaving CAB, you may take these
benefits with you to your new employer.

Employee Assistance Program (EAP)
CAB offers a work-life balance employee assistance program for
all of its employees. Lincoln Financial provides a toll-free 24 hour
assistance program, as well as on-line references to help find
solutions for the everyday challenges of work and home as well
as for more serious issues involving emotional and physical
well-being."
Good News! As a CAB employee, we sometimes have access to group
discounts to certain stores, restaurants, movie theaters and
amusement parks.
